Saving Appellate Court Forms

The Judicial Council is pleased to announce its court forms can now be saved to your computer, "[t]hanks to the efforts of the Superior Court of Orange County" and its PJ, Judge Kim Dunning.

Several appellate forms are available, including:

• APP-002 Notice of Appeal/Cross-Appeal (Unlimited Civil Case)
• APP-003 Appellant’s Notice Designating Record on Appeal (Unlimited Civil)
• APP-004 Civil Case Information Statement
• APP-008 Certificate of Interested Entities or Persons
• APP-009 Proof of Service (Court of Appeal)
